If you are planning to build or repair a septic system, it is required by law that a permit be obtained from the Health Department. The purpose of the Health Department involvement is to ensure protection of the public health, protection of our groundwater and to provide you with the best information available about the design of a sewage treatment system for your property.

To avoid any delay in processing your permit application, please fill out the form completely. A detailed sketch of your proposed property layout of well, sewage treatment system and home location will help us design a plan that does not interfere with your building plans. If help is needed to complete the application please contact your Local Health Department Office.

Once an application is made, a sanitarian will visit the property to evaluate the soils and site suitability for a sewage treatment system. With your lot plans and building plans in mind, the sanitarian will design a sewage treatment system best suited to your needs and soils conditions.

We ask you to prepare for the sanitarian’s visit with the following steps:

1. Mark property boundary lines to be easily located. Post sign at road identifying property.
2. Insert a stick in the ground with flagging attached next to your drive proposed area for sewage treatment system and well. This will help assure the sanitarian is at the proper address.
3. Confirm and Mark the location of your water supply as well as neighboring water supplies with flagging tape.

Once the site evaluation is completed, the Health Department will contact you or mail the approved permit to the mailing address listed on the application. If you have any questions about the permit specifications, you should contact the Health Department before beginning any work on the site.

Your Local Health Department Branch Office Staff would like to thank you for your cooperation.

1. Please fill out a Water Supply Permit application and pay the fee by mail or in the office.
2. A site visit may made by an Environmental Health Sanitarian to evaluate the site conditions.
3. The permit is completed and mailed to the owner so that any special specifications and/or instructions can be followed.
   
   WITHOUT ANY UNFORSEEN PROBLEMS OR DELAYS THIS PROCESS SHOULD TAKE 1-2 WEEKS.
   
   
4. When well log is received and the well is connected to the home, the owner or installer must call the health department to schedule an appointment for a well inspection and water sampling.
5. A Sanitarian then visits the site to inspect the well construction and take sample(s) of your water for bacteria. These samples are sent to the lab for analysis. It takes about 7 to 10 days to receive the results of the water sample from the lab.

Final approval of your Water Supply will depend on the results of your bacteria water sample and construction of your well. Copies of the permit are available upon request.
