Families, who reside in Arenac County, and who have children ages 0-3, can be enrolled in the Success
by Three Program upon being referred by Michigan Department of Human Services personnel, Arenac County
office. Referrals are made to the program when it has been determined that infants and/or children are
at risk within their living environment.
Through an interview / assessment process, offered to parents by Success by Three Program personnel,
an "Individualized Service Plan" [ISP] is completed. The ISP includes, but is not limited to, such
aspects as an assessment of family needs, and referrals to Arenac County human service agencies.
Once enrolled in the Success by Three Program, the family receives home visits over a specified
period by a Public Health Nurse or a Health Educator. The visits will primarily focus on the goals
established within the ISP, support to the family, the promotion of healthy parent-child relationships,
and the assurance of healthy child development. The staff utilizes the "Parents as Teachers" curriculum
as a means to support and preserve the family unit.
