SUCCESS BY 3 - CLARE

Families, who reside in Clare County, and who have children under the age of four, can be enrolled in the Success by Three Program upon meeting eligibility requirements and contacting the program coordinator. Oftentimes families are referred to the program through the efforts of county agencies and organizations when there is potential for a child/children to be at risk within the home environment.

Through an interview / assessment process, offered to parents by Success by Three Program personnel, a determination is made as to whether the family meets the program eligibility requirements and if the family will be appropriately assisted by enrolling in the program. Once enrolled in the Success by Three Program, the family receives bi-weekly visits by a Health Educator and/or Public Health Nurse. Home visits are based on goals and objectives identified by the parents, and with the assistance of program personnel, an "Individualized Family Service Plan [IFSP]" is created. Each child is also assessed utilizing the "Infant Toddler Developmental Assessment [IDA]", allowing potential developmental delays to be identified and addressed through early intervention.
