Immunization Information
*Attention* If you have children attending a preschool program or licensed child center
To attend a nursery, licensed child care center, preschool, Head Start Program,
Pre-primary Impaired, or 4's at Risk Program: *State law allows a director to
prohibit admittance of a child to the program without a record of age appropriate
immunizations.
You must provide the
program director with a record showing that your child has received all of the following
immunizations, 1 - 2 times during the year.
To Enter School: State Law allows a principal or teacher to
prohibit the admittance of new entrants to school without a record of having received age
appropriate immunizations.
To Stay in School:
You must provide the school with a record showing that your child has received
all the following immunizations:
