Vaccines have contributed to a significant reduction in many childhood diseases, such as diphtheria, polio,
measles, and whooping cough. It is now rare for American children to experience the devastating effects of
these illnesses. Infant deaths due to childhood diseases have nearly disappeared in the United States and
other countries with high vaccination coverage. But the germs that cause vaccine-preventable diseases and
death still exist, and can be passed on to people who are not protected by vaccines.
The Central Michigan District Health Department in conjunction with the Michigan Department of Community
Health provides age appropriate vaccinations and health education. Vaccines are available for all ages
including flu vaccinations. We offer vaccinations to those with and without medical/insurance coverage.
No immunizations are kept from children based on the ability or inability to pay.

Application Process: None, Appointments are necessary. Persons aged 18 years and older can provide consent for themselves.
Documents Required: Bring your immunization record with you to your appointment.
Fees: Check with your insurance company before your appointment to see if your plan covers immunizations. Some insurance plans can be billed for certain vaccines. Contact the local branch office for more information.
Payment Method: Payment can be made by cash or check.
Hours: Office hours are Monday – Friday, 8:30 a.m. – 12 noon; 1:00 – 4:30 p.m.
Languages: English, Interpreters are available – please notify us in advance if you need this service.
Service Area: Arenac, Clare, Gladwin, Isabella, Osceola, Roscommon Counties.
