Join our team as a Community Health Worker!

Seeking a Full-Time position for COMMUNITY HEALTH WORKER (CHW)

The position is: 1.0 FTE (80 hours/pay) for Roscommon County for the Central Michigan District Health Department to support Personal Health Services and act in a liaison role with our Community Connections Program. No specific county has been assigned until after hiring, to attract the best candidate.

 

The CHW’s at CMDHD work in a team-based environment and will spend time in the community with many local agencies and community services providers primarily in person or in an office engaging directly with clients assisting with Social Determinates of Health (SDOH) needs. This includes but is not limited to helping community members with individualized support with Medicaid Health Plans and work with the Community Connections Program as part of the Community Health Innovation Region (CHIR). This is a rewarding opportunity to work with medically complex patients who may be experiencing significant health related concerns, preventative care needs, addiction and/or behavioral health conditions or other factors influenced by their social determinants. In addition, this position will be required to complete reports, track data, and while paying careful attention to details.

 

  • Experience as a Community Health Worker is required and certification as a Community Health Worker is highly desired. Certification training is available for the right candidate.

  • Experience working with community resources, low-income families, and/or the Medicaid population preferred, with preference given to those with years of experience in this role.

  • Education equivalent to High School diploma and a minimum of two years’ experience working in the public health or health care is required.

The ideal candidate will be detail oriented and possess exceptional technical skill working with an electronic health record (EHR) system. CMDHD offers a comprehensive benefit package including the ability for a flexible work schedule. Hourly wage $15.06 – $20.04. EMPLOYMENT VACANCY


Central Michigan District Health is seeking an Account Clerk, Billing Coordinator, 1.0 FTE (40 hours/week), to support the medical billing and accounts receivable functions of the Agency. This position includes monitor of the AR, employee education, maintain credentialing of all professional staff, resolve disputed insurance claims, serves as a liaison for supervisors and employees to identify and resolve billing issues and processes, performs billing audits, and prepares financial reports. This position will be based in the Main Office. Possession of an associate degree in Medical Billing or its equivalent in terms of education, training or experience, required. We welcome you to join our team as a Registered Dietitian (0.8 FTE, 32 hours per week) position in the Arenac and Roscommon County Branch offices of the Central Michigan District Health Department, improving the health outcomes of women and children in the community.  For the right candidates, this position may be separated into two part time positions.  This position is an integral part of our public health team serving both the Women, Infants and Children (WIC) Program and the Maternal and Infant Health (MIHP) Program, providing nutrition education, support, and direction to families. Having excellent communication skills to accommodate our client’s health related needs and concerns, while being detail oriented and possessing exceptional technical and counseling skills are necessary. Opportunity for utilization of new technologies in telehealth may be available. A Bachelor of Science Degree in nutrition and with current registration as a Registered Dietitian required.  One year experience in community health nutrition or outpatient counseling recommended.
