We are asking community members to fill out two surveys about COVID-19 and emergency preparedness in our communities.  These surveys were created with the assistance of our county emergency management partners and will be open until July 22, 2022.  At the end of the survey, you will have the option to enter your contact information for chance to win a $25 Amazon gift card.  One winner will be chosen in August 2022.

 

Survey results will be shared within Central Michigan District Health Department and with our county emergency managers The survey results will be included in a final report written by a Master of Public Health Student from Central Michigan University to be used as an internship project in order to obtain the degree.  Any names, emails and contact information will be removed in the final report.
