Community Connections

What is Community Connections?
  • It's a FREE program offering connections to community resources for adults, children, and families.

  • Services are provided through phone calls, home visits, and office visits.

  • Services are delivered by a Community Health Worker.


Who is eligible?

Any adult, child or family living in Arenac, Clare, Gladwin, Isabella, Osceola, and Roscommon Counties.

 

 

 



How does Community Connections help adults, children, and families?

Individuals will get help with access to medical care, transportation, food, utilities, education, classes and more! Participants may also get home visits from a Community Health Worker. Health care providers assist with:

  • Medical, dental care or mental health

  • Finding you a doctor or dentist

  • Finding transportation

  • Enrolling you in health insurance

  • Answering questions about wellness or being sick

  • Getting food or a place to live

  • Buying, fixing, and eating healthy foods

  • Changes that you decide to make to be healthy, such as quitting or cutting down on smoking, alcohol, or drugs

  • Your concerns about abuse or violence in your life

  • Answering questions about family planning (birth control)

  • Finding translation services

  • Baby shots/immunizations

  • Connecting you with community services, like GED education classes, employment opportunities, food pantries that give out food for free, the WIC food program, or heat and electricity resources
