HIV/AIDS Treatment Programs
Central Michigan District Health Department Ryan White Programs provides care, medication, and essential support services to people living with HIV/AIDS to improve HIV-related health outcomes, reduce HIV-related disparities and health inequities, and prevent new HIV infections. CMDHD offers both infectious disease and primary care management for people living with HIV/AIDS. CMDHD will not exclude from participation in, deny benefits of, or discriminate against any individual or group because of age, physical or mental challenges, creed, criminal history, history of substance abuse, immigration status, marital status, national origin, race, sexual orientation, gender identity and expression, socioeconomic status, and current/past health condition(s).
Eligibility
To be eligible for the Ryan White Services, an applicant must meet and provide proof of the following:

Documentation of HIV positive status/diagnosis

Must reside in the state of Michigan

Must have an income ≤ 500% of the most current Federal Poverty Level
-
Must be underinsured or uninsured for applicable Ryan White services that are reimbursable through third party payers

Infectious Disease and Primary Care clinics

Mental Health screening, assessment, diagnosis, treatment, and counseling

Nutritional Therapy screening, assessment, education, counseling, and nutritional supplements

Medical Case Management offers assistance with applications for housing, food assistance, and transportation, as well as enrollment in private and public programs such as Medicaid, Medicare Part D, and State Pharmacy Assistance Programs.

Tobacco Cessation

Support Services including Food Assistance, Medical Transportation, and Emergency Financial Assistance

Michigan Drug Assistance Program (MIDAP) - MIDAP covers HIV-specific and HIV-related prescription medications, as well as vaccines, for eligible Michigan residents with HIV. MIDAP also covers hepatitis treatment for eligible individuals co-infected with HIV and hepatitis C. - MIDAP FORMULARY
