Information for 5-17 Year Olds

A parent must accompany a 5-17-year-old to a clinic to be vaccinated. For CMDHD, if a parent cannot be there, someone older than 18 must still accompany the child and they must bring the filled-out form below.

Michigan residents vaccinated outside of Michigan – How to make your vaccination count in Michigan

Work with your healthcare provider to ensure immunization data is added in the Michigan Care Improvement Registry (MCIR). It’s best to contact your provider before visiting their office. You will have to take your COVID-19 Vaccination Record Card to your provider’s office. Click here for more information.

Call your local health department branch office to set up a time to bring your COVID-19 immunization record into of our offices to have it added to your existing immunization record in the Michigan Care Improvement Registry (MCIR).

Lost, unable to find or never received COVID-19 vaccination record card

Contact the provider (medical provider, local health department or pharmacy) where you were vaccinated and request either a new COVID-19 Vaccination Record Card or a copy of your current immunization record from the Michigan Care Improvement Registry (MCIR).

Visit https://www.mcir.org/public/ to request an official immunization record, which will display proof that you have received a COVID-19 vaccine.

Please note: The COVID-19 Vaccination Record Card and the official immunization record from MCIR are official vaccination records.
